Job Description:
Overview$10,000 Sign on Bonus UnityPoint at Home - Waterloo,
IowaSocial Worker MSWFull time Days - Monday-Friday, 40hrs a
weekResponsibilitiesMaintains the standards of social work
excellence in accordance with the scope of practice and standards
of care set forth by the proper state chapters of Social work and
the NASW Standards for Social Work Practice in a Health Care
Setting. Patient CareConducts assessments, diagnosis and treatment
of the social and emotional factors related to the patient's
illness, need for care, response and adjustment to care and
completes appropriate plan of care and interventions according to
scope of practice. Care CoordinationConsistently utilizes a
holistic approach considering physical, psych/social, spiritual,
educational, safety and related criteria appropriate to the age of
the patient. Participates in the development of the plan of care
and attends interdisciplinary team meetings. Clinical
ExcellenceAssesses the relationship of the patient's medical and
nursing requirements to the patient's home situation, financial
resources and available community resources. Provides information
and appropriate referral action to obtain available resources to
assist in resolving patient/family issues. Direct & Indirect
Supervision of Social Workers to review documentation as needed.
Provide mentorship supportQualificationsEducation: Master's degree
in social work from a University/College accredited by the council
on Social Work EducationExperience: One year clinical experience in
health care. Previous hospice and home care expreince is preferred.
License(s)/Certification(s):State Licensure per State CodeValid
driver's license when driving any vehicle for work-related
reasons.Mandatory Reporter: Child & Dependent Adult AbusePerson
Centered Care (PCC) course completion within first 12 months of
hire and annual completion of competency validation
activities.State Licensure per State Code.CPR
CertificationKnowledge/Skills/Abilities: Strong interpersonal
skills and customer service skills.Strong verbal and written
communication skills Ability to take initiative and exercise
independent judgment, decision making and problem solving
expertise.Ability to work as a collaborative team member. Ability
to understand and apply guidelines, policies and procedures.Lift
Requirements: Exerting up to 50 pounds of force occasionally,
and/or up to 20 pounds of force frequently, and/or up to 10 pounds
