You'll evaluate and treat clients with physical and emotional limitations in the comfort of their own homes-designing therapy plans tailored to their goals and environment.Key Responsibilities

• Perform timely home evaluations and develop customized treatment plans based on physician referrals and patient needs.
• Set functional, measurable goals and modify therapy based on patient progress.
• Educate patients, families, and caregivers on therapy, safety, and exercises to enhance recovery and independence.
• Collaborate with interdisciplinary teams and participate in case conferences to ensure whole-person care.
• Maintain accurate documentation in compliance with home health standards and federal regulations.
• Adhere to MercyOne's safety and infection control policies.What You Need to Succeed
  • Licensed Physical Therapistin the state of Iowa.
  • Master's degree in Physical Therapy(Doctorate preferred).
  • ValidIowa driver's license, insured vehicle, and reliable transportation.
  • BLS certificationand Mandatory Reporter training required.
  • 0-2 years' experience in physical therapy (home health experience preferred).
  • Strong communication, time management, and independent problem-solving skills.What Sets You Apart
