Dedicated Care Team to assist with care gaps, patient outreach, care management, post-ER and admission monitoring, post-discharge follow-up care, and RX support.

$50,000 Signing Incentive. 3 weekday shifts per workweek and 1 weekend (Saturday & Sunday) per month; total of approximately 144 shifts per year. Weekday shifts - 7am to 7pm; Weekend shifts - 8am to 3pm. Participate in management of the department, including covering open shifts as needed. Friendly collegial atmosphere. Excellent payor mix.
